	Name and Address Change Form (06/02) (70KB, PDF format)	This form is for lessees and permittees of the Land Division of the DLNR to request an address or name change. IMPORTANT: Only the legal lessee or permittee may sign and submit the request.

	Shoreline Certification - Notice of Appeal (05/2003) (44KB, PDF format)	This Notice of Appeal form is for persons requesting an appeal of a proposed shoreline certification or rejection. Appeals must be received no later than 20 days from the date of OEQC public notice of the proposed certification or rejection. Please refer to §13-222-26, HAR, for the rules on appeals of shoreline certification.
