ORDER AMENDING FORMS

IT IS HEREBY ORDERED that the attached Forms #DC27 and #DC27C are amended for use in the District Courts of the State of Hawai‘i, effective July 24, 2009. The amended forms replace the prior forms bearing the same "DC" number;

IT IS FURTHER ORDERED that the district courts are authorized to (a) insert circuit identifiers, addresses, and contact information and (b) publish the form in print or electronic format.
